I was recently surprised to see that this is in fact not enabled in Freeciv as it was in original Civ(s), not even as a ruleset or a server setting. So:
Please include the number of polluted tiles at the end of the game into the end score. Civ 1 reduced the final score by -10 for each polluted tile but maybe it would be good to make it possible to set this value through server or ruleset setting. This should also make it possible to disable it completely for incorrigible polluters.

Maybe makes pollution/fallout made people ill. Additionally, number of ill people will decrease score. Another option is pollution/fallout makes people less happy.

I'd go further and suggest an option which makes it impossible to win the game as long as there is even just a single polluted tile
